New Patient Registration
We are happy to register patients who live within our practice area. We serve the villages of:-
You can collect a new patient pack from the surgery.
Please complete the documentation and return to the surgery along with two forms of identification, a photographic form of ID (such as a passport or driving license) and proof of your home address (such as a recent bank statement or utility bill).
As a new contractual agreement, we are now obliged to inform all new patients registering with The Old School Surgery of your allocated named accountable GP, this will be allocated to you once you have been registered with the practice in writing. This is purely an administrative procedure. We have an open policy here for patients, you may make an appointment to see whichever GP you wish.
In addition, if you are aged 75 or over you will also be notified, in writing, of a named GP who will have overall responsibility for the care and support that our surgery provides to you.
